Register

Requirements:
To register for Sparky's Toy Drive LaSalle, please fill out the form below. We'll contact you to arrange to verify the following required documentation and to provide your appointment time on distribution day:
1. Identification: The driver's license of the parent or guardian of the children for whom you are registering.
2. Proof of Residence: For example, a rent receipt, utility bill, or some other documentation showing your address.
3. Health Cards and/or Birth Records: Originals of the health cards or birth records of the children for whom you are registering (must not be photocopies).
4. Income: Bring documentation that shows your total family income with a cheque stub from one of following: a) Ontario Works (OW), b) Employment Insurance (EI), c) Ontario Disability Supplement Program (ODSP), or d) Your employer (last year's Income Tax T4 statement); and your Canada/Ontario Child Benefit Notice for the current year.
